CSA’s Senior Case Management program provides wrap-around services to help seniors live in their own homes for as long as possible and remain connected to their community. Our geriatric case managers help clients manage their health and navigate available social services, providing personalized 1:1 support along the way.
“Our clients are like family,” says Case Manager Lisa South. “In partnership with CSA volunteers, we help ensure that vital needs are met – whether it be rides to appointments, writing down doctor’s instructions, picking up prescriptions or help with grocery shopping.”
Jaemie Abad, RN Case Manager, Intensive Case Management, echoes this sentiment. “CSA believes in empowering individuals and their families through education, referrals and advocacy. This way we can encourage our clients to be as happy, healthy and safe as possible while respecting their autonomy and dignity.”
Our senior clients come from many backgrounds and circumstances. Case in point: earlier this year, the Mountain View Fire Department contacted CSA about a newly widowed former teacher whose late spouse had had a collecting behavior. Due to injuries sustained from a car accident, the client was not able to cook, drive or shop for groceries. CSA helped the client contact Meals on Wheels for food needs, Rebuilding Together to make home repairs and a local recycling agency to help dispose of the accumulated items. CSA also arranged physical therapy home visits. Through all these interventions, the client was stabilized and went on to help out as a volunteer at a local children’s summer program.

“I am grateful to have the opportunity to contribute something for the greater good,” says Case Manager Hazel Guevarra. Adds colleague Lisa Vergati, “My work makes me feel like I am part of something bigger than myself.”
